Although an April study suggested that genetic variations influence individual drug responsiveness and clinicians report "super responders" achieving success on reduced doses, professional organizations like the American Association of Clinical Endocrinology urge strict adherence to FDA-approved guidelines due to insufficient trial data. Self-directed microdosing also introduces physical hazards: utilizing syringes to extract medication risks contamination, while counting pen "clicks" is highly error-prone and often extends pen usage beyond its intended 30-to-60-day shelf life.

While medically supervised microdosing allows doctors to monitor patient health and adjust maintenance doses safely, unguided reduction may undermine the broader therapeutic spectrum of GLP-1 therapies. Clinical trials validating secondary health benefits, such as enhanced cardiovascular health, relied exclusively on full dosage protocols, leaving lower doses unproven. Furthermore, pharmaceutical manufacturers lack financial incentives to fund trials proving that lower product consumption is effective. In the absence of rigorous clinical data, individuals contemplating self-directed dose reductions are advised to exercise caution and adhere strictly to professional medical guidance.
