In the Democratic Republic of Congo’s fast-moving Bundibugyo Ebola outbreak, the scientific community has mobilized with unusual speed: a little more than 1 week after the WHO declared an emergency, experts had already narrowed down the most promising treatment and vaccine candidates and had a workable plan for testing them. Yet no vaccine or treatment is currently authorized for this strain. The main options on hand are tools developed for Zaire Ebola, including remdesivir, its oral version obeldesivir, and an antibody cocktail being developed by Mapp Biopharmaceutical, which the WHO, the DRC and Africa CDC have prioritized for testing.

The outbreak is colliding with severe on-the-ground constraints. Suspected cases are nearing 1,100 and suspected deaths are close to 240, while Tedros Adhanom Ghebreyesus said parts of the DRC face “a catastrophic collision of disease and conflict.” Aid workers and health facilities have been attacked, and misinformation about Ebola plus distrust of humanitarian help are widespread. That makes even basic tasks such as isolating patients and tracing contacts difficult, let alone running a clinical trial.

There are still scientific reasons for guarded optimism. The Bundibugyo and Zaire glycoproteins are about a 70% match, so some Zaire vaccines or therapies may work against Bundibugyo; remdesivir performed poorly in human Zaire studies, but lab tests suggest stronger activity here. A Global Preparedness Monitoring Board report said preparedness is being outpaced by pandemic risks, geopolitical instability and rapidly evolving information ecosystems after nearly a decade of health emergencies. In this outbreak, the gap between scientific progress and social reality has become decisive.
