Health care has once again emerged as a central electoral vulnerability for Republicans ahead of the midterm elections. According to a July poll by KFF, more Americans expressed anxiety over paying for health care than for petrol, housing, or groceries, while a March Gallup survey found that 61% of Americans worried a great deal about health-care availability and affordability, surpassing concerns about the wider economy and inflation. Unlike during the 2018 midterms when Democrats mobilised to protect the Affordable Care Act (ACA), the law is currently unravelling following Republican cuts to federal insurance exchange subsidies and the Medicaid program for low-income citizens.
Systemic cost pressures have intensified alongside legislative retrenchment. Insurance premiums rose at their fastest pace in 15 years in 2026, propelled by expensive breakthrough treatments for cancer and obesity alongside AI-driven billing software deployed by hospital networks to maximise charges. As notices arrive for the half of the population insured through employers, consultancy Marsh projects that employer-sponsored premiums will surge by an average of 8% for 2027. Seizing on this economic strain, Democrats have poured $93m into health-care campaign advertisements across competitive congressional districts since January, making it their most heavily funded campaign theme.
Republican efforts to reframe Medicaid retrenchment as combating welfare fraud have failed to sway the electorate, and Donald Trump's September pledge of $500 "refunds" to nearly 1m citizens dropped from exchange subsidies leaves 3m former participants who lost coverage entirely unassisted. A New York Times/Siena College poll indicates that 60% of voters trust Democrats on health care, while Cook Political Report data reveals that 56% of swing-district voters fear benefit reductions far more than fraud or waste. Even as internal progressive and moderate debates simmer over single-payer versus incremental ACA repairs, Democrats are counting on soaring medical bills to secure a November victory.
