Autism is a spectrum disorder, and about 3.2% of American children are on it, with severe cases sometimes unable to speak or care for themselves while milder cases often remain in mainstream schools despite communication and emotional-regulation challenges. Diagnoses have quadrupled over the last 20 years, and a Wisconsin epidemiology study found that between 2000 and 2016 the increase was concentrated in mild to low-adaptive-impairment categories while moderate-to-severe cases stayed mostly flat, alongside higher rates of gastrointestinal, obesity, sleep and seizure comorbidities.

Demand for care has turned autism services into a multibillion-dollar sector; after Medicaid coverage rules were clarified in 2014, visits to Applied Behavior Analysis (ABA) centers rose from just under 7 million in 2019 to 28.5 million in 2025. State costs escalated sharply: North Carolina rose from $121.7 million in 2022 to a projected $1.1 billion next year, Nebraska saw a 2,560% rise in visits and Medicaid spending from $4.6 million in 2020 to $85.6 million in 2024, while Colorado audits begun in December found $285 million in questionable payments, and the share receiving more than 10 care-hours weekly doubled to 58% with average visits per patient up 17% nationwide from 2019 to 2025.

From 2017 to 2022, private equity dominated consolidation, accounting for 85% of autism-sector M&A; a Brown University study identified 574 therapy centers owned by PE firms by 2024, and in Colorado half of 2024’s billable-hour growth came from PE-backed providers. Although controlled studies show ABA can improve language, socialization, and daily functioning, more hours are not necessarily better, and the workforce has expanded from 328 registered behavior technicians in 2014 to 246,000 last year, where low pay and turnover undermine continuity, while autism’s heterogeneity means children often need coordinated speech, occupational, and pediatric nutrition/medication management that is still too rare.
